Essential Functions

  • Perform initial client assessment for home care

  • Travel to patients' homes to provide medical and personal care.

  • Develop individualized care plan for the client, in coordination with their primary care doctor, insurance care coordination team and other valued partners. 

  • Writes a custom nursing care plan based on physical assessment, risks in the patient’s current environment, medication review/education, and management of any durable medical equipment required in their care.

  • Contacts the supporting physician for any new orders that might be required related to a variation from a normal assessment. 

  • Educate clients about their disease process and prevention, medications, and general safety. Also educate clients about when to seek help from their medical team and when to pursue hospital treatment.

  • Use medical equipment to monitor the patient’s vital signs known as telemonitoring or telehealth.

  • Keeping accurate records of services performed, patients' health status and hours worked.

  • Administering medications and providing other basic health supports such as inspecting wounds and changing dressings.

  • Test for infection, weakness and bed sores.

  • Provide clinical direction to caregiving staff. 

Qualifications

 

  • Active, unencumbered Indiana Registered Nurse (RN) license

  • At least 3 years of RN experience required

  • Previous experience in home health setting required 

  • Pass a background check and BMV screening

  • Valid driver's license, reliable transportation and auto insurance

  • TB test results taken in the last 9 months

  • Valid and active CPR certification

  • Successfully complete new hire onboarding & orientation
